NEW GOVERNANCE RULE:
- ID: inst_062
- Text: "GitHub README.md must be reviewed weekly and 'Last Updated' date updated when material changes occur"
- Quadrant: OPERATIONAL
- Persistence: HIGH
- Temporal Scope: PERMANENT

RATIONALE:
GitHub README is the primary external interface for implementers.
World-class open source projects maintain current, accurate README files.

MATERIAL CHANGES (require date update):
- Service additions/removals
- Architecture changes
- Status updates
- Documentation links
- Critical corrections

NON-MATERIAL CHANGES (no date update):
- Typos
- Formatting
- Minor clarifications
